NORTHWEST FAMILY PHYSICIANS HAS ADDED TWO NEW PHYSICIANS TO THE CRYSTAL OFFICE.
Gretchen Soberay, D.O. joined Northwest Family Physicians October 5th, 2009. Dr. Soberay is a recent graduate of University of Minnesota North Memorial Family Medicine Residency Program. She practices the full range of Family Medicine and Obstetrics.
Jennifer Riley, M.D. started in January of 2010. Dr. Riley is a graduate of the University of Minnnesota School of Medicine and the North Memorial Residency program. She enjoys the full range of Family Medicine with special interest in OB, women’s health and preventive medicine.
Colonoscopy
Colon cancer screening with a colonoscopy is now available at both our Plymouth and Crystal offices. A colonoscopy is recommended starting at age 50 and is an effective exam for detecting pre-cancerous polyps or colon cancer at an early treatable stage.
With many insurance plans now having high deductibles and co-insurance it is important for you to know that having a colonoscopy in a clinic office setting is significantly less costly that if it were to be preformed at a hospital.

DIAMOND Depression Care
As of March 2010, Northwest Family Physicians now offers DIAMOND care for patients with depression at all three clinics. DIAMOND stands for “Depression Improvement Across Minnesota, Offering a New Direction.” This method of depression care changes the way care is given and paid for in primary care. DIAMOND patients are cared for by a team. The care team includes your primary care provider, a care manager, and a consulting psychiatrist. Patients meet with their care manager as often as necessary. The care manager meets with a psychiatrist weekly to discuss any recommended changes in treatment which are then discussed with the primary care provider. If you suffer from depression, or think you may have depression, please talk with your provider about DIAMOND.

Platinum Blue
Northwest Family Physicians has joined the Blue Cross Blue Shield Platinum Blue senior insurance plan. The contract is currently in place and we are accepting this insurance at all of our locations.
Weight Management
Don’t weight anymore! Northwest Family Physicians is offering group visits monthly on the topic of weight management. Register now for a group education session focused on weight management. Each session will include group education, and individual assessments and goal setting. The sessions will be led by Tammy Gregerson, P.A.-C.
